Just so you are aware all our calls are recorded for training and monitoring purposes.
ANPR parking with vehicle number plate recognition is in operation 24hours a day 7 days a week at both our sites, therefore we require all patients to register their car at the surgery during all visits to avoid any fines. There are two access points to register your number plate at Cressex and one at Lynton.
To register with Cressex Health Centre
Complete the forms below and return to the surgery – the welcome page in the registration pack provides instructions of documents required. If you require Online access, please ensure your photo I.D is verified by the receptionist before you leave, and that this is signed off otherwise we cannot issue.
If you cannot attend the surgery, you can email us your registration (email below) this only applies if you were previously registered with a GP practice. If this is your first registration (new to the NHS) this will be rejected without a reply, this is so we know you are residing in the country. For first time registrations you are required to attend in person.
Please ensure during completion process to provide your NHS number, this helps us locate your record on the database and prevents any duplication errors which may later affect your care. If you do not know this you can find this here or you can contact your previous practice who may ask you a few security questions to ascertain your identity.

FAQ
How do I register?
Registration forms are available via the links on this page (see above) just print, complete and return to either site Cressex Surgery or Lynton surgery. We require both the GMS1 and questionnaire to be completed.

Do I have access to both surgeries?
Yes; however this is dependent on clinics running on that day and opening times. We cannot guarantee you will be seen at a site of your choice, but will do our best to accommodate you.
How long does it take to register?
Registrations can take up to 3 weeks and in busier periods, up to 4 weeks. However if your registration is urgent let us know and we can prioritise this for you.
Who would my named GP be upon registering?
Named GPs are assigned to each patient upon registration, this is completely random and does not affect your care. For further information click here
My registration is urgent can I be registered today?
This depends on the nature of the problem and our capacity on the day, please call us to discuss and we will do our best to help you.
I am out of your catchment area, however I work in the area or have other links to the area, can I register?
This largely depends on the distance and your current state of health. There are many factors to consider to ensure safe practice including but not limited to onward referrals and home visits – which do not apply to out of area patients. Please call us to discuss.
I am pregnant, what is the process for registering and seeing a midwife?
Inform reception that you are pregnant when handing in your completed registration forms. Midwife referrals are paperless click here to self refer. The midwife services are currently delayed, they aim to be in touch at around 10 to 12 weeks.
Can I register for patient access at the same time as my registration (for age 13 and over)?
For access to your online record and ability to request repeat medication please refer to the NHS app, this requires no interventions from the GP surgery, if you have a query regarding this then refer to technical support via the app – we do not manage this.

I am in the area for a short period; can I register as a temporary patient?
Please be sure which service you require before contacting us in the first instance:
- Emergencies 999 or Stoke Mandeville Hospital for nearest A&E

We cannot always guarantee temporary registrations, this largely depends on our capacity on the day and priority to our registered patients. If your issue is immediate and necessary get in touch and we can liaise with management and Duty Doctor to see if a temporary registration is appropriate. It may also be there are other services that are more suited to your needs.
Unfortunately we are not a walk in centre and do not take referrals from other care providers.
I need to be seen urgently but the GP surgery is closed?
If your problem cannot wait please call 111 for the out of hours service which kicks in from 6.30pm when the surgery closes.
