Data Opt-out

Please take time to read this information – you need to make a choice.

NHS in England is introducing a modern data service on behalf of the whole health and social care system that will combine data from hospitals and GP surgeries. This information can help improve understanding, locally and nationally, of health needs and quality of treatment provided by local health services. The data will be held in a secure system and shared with researchers and planners to shape decisions about future health care resources. The sharing of information will be restricted on a case by case basis and only the minimum amount of information needed for that particular case will be shared.

Data will be extracted from our clinical records every month and will include a patient’s postcode and NHS number, but not their name.

Opt-out deadline has been extended until 30th September 2021.

Briefly there are two types for Opting-out:

Type 1 – Opt out 1

This is concerning primary care with your GP Practice

If you do not want your identifiable patient data (personally identifiable data) to be shared outside of your GP practice for purposes except for your own care then complete Opt out 1 and return to us.

Type 2 – National Opt-out.

This is concerning sharing hospital data (secondary care) and now GP data from 1st October 2021.

You can Opt-out here. Before you start you must:

  • be aged 13 or older
  • have access to your email or mobile phone
  • have your NHS number or your postcode registered with your GP surgery
